Patient Profile Overview

Learn how to view resident details, manage contacts, and send education from an individual patient profile.

Summary

The Patient Profile gives you a complete view of a resident’s information, diagnoses synced from PointClickCare, contacts, and document activity. From this screen, you can also send education directly to the resident or their contacts.


Main Points

Accessing a Patient Profile

  1. Click the Patients tab on the left navigation menu.

  2. Select the resident you would like to review.

This opens the individual Patient Profile.


Patient Details Section

At the top of the profile, you will see:

  • Resident name

  • Date of birth

  • Admission date

  • Email address

  • Mobile number

This information is synced from PointClickCare.


Diagnoses and Treatments

Below the resident details, you will see:

  • Diagnoses

  • Treatments

These are identified and synced directly from PointClickCare. This information helps guide appropriate education and documentation.


Contacts

The Contacts section displays:

  • Resident contacts

  • Contact titles or roles

  • Contact information

From this section, you can:

  • Call a contact

  • Send an email

  • Send documents directly

This allows quick communication and education delivery without leaving the profile.


Documents in the Resident Portal

The Documents section shows all education and documents associated with the resident.

You can view documents by category:

  • Requires Action
    Documents that need a signature or other required action

  • Completed
    Documents that have been signed or finalized

  • All Documents
    A full list of documents sent to the resident

You can scroll to view additional documents as needed.


Send Education from the Patient Profile

At the top of the Patient Profile, you can send education directly to the resident.

  1. Click the Send Education button.

  2. Choose to send:

    • A full report containing all current documents

    • Additional documents from the Library

    • Documents using the search feature

  3. Select the appropriate materials.

  4. Click Save and Continue.

  5. Complete the sending process.

This allows you to quickly deliver personalized education and document disclosure directly from the resident’s chart.
